At The Hall Practice we offer an annual review to people with the following long-term conditions:

• Asthma
• COPD
• Coronary Heart Disease
• Diabetes
• Dementia
• Epilepsy
• Learning Difficulties
• Mental Health Conditions
• Stroke / TIA

The review will usually be offered in your month of birth and you will receive a letter from the practice when it is due.  If you think you are due a review and have not heard from us, please call the Nurses' secretary on 01753 278948 (8.30am - 12.30pm).
