Hospital changes to medication

If following a hospital admission you require additional medication, they will normally provide a two week supply of medication. If you need this medication to continue, our pharmacy team will need to review a copy of your discharge summary containing the medication details. We should receive a copy of this electronically from NHS hospitals. 

 

If your medication has changed at the request of a consultant or a recent hospital appointment, our pharamcy team need to review a clinic letter clearly documenting the medication required and the clinical reason. We do receive notification from hospitals/specialists about changes, however you may need to contact your specialist to issue the altered medication if you need to start it urgently before this information arrives at the surgery.
